Repeat Prescriptions
From 2nd August 2010 a new 24hr
Repeat Prescription Ordering Line will be in operation in the
Surgery:
- Call 028 3832 2189
- Leave your name, address, date
of birth, telephone number, patient ID (if known), names and
doses of drugs (spelling difficult names when necessary)
Computer generated prescriptions have a
tear-off segment on the right hand side. This contains a complete list
of your current medications.
Please keep only the up-to-date list and discard any
previous lists each time you obtain a prescription.
You can use the tear-off list when ordering repeat prescriptions.
Just put a tick ( )opposite the item you require on the list and drop it
into the surgery reception. This will avoid mistakes and
misunderstandings about the drugs you require repeated.
Repeat prescriptions may be collected from reception
but leave 2 full working days before collecting and if ordering
by telephone at the weekend, collect on Wednesday. |
